How to Complete the EK-2 Pre-Employment and Periodic Examination Form

The sections of the EK-2 form, the fields completed by the employer and the occupational physician, the conclusion section and retention periods. A checklist for inspections.

The EK-2 form is the standard document for pre-employment and periodic health examinations. It is annexed to the regulation on the duties, powers, responsibilities and training of occupational physicians and other health personnel, and it is the written output of health surveillance under Article 15 of Law No. 6331. Inspectors look for one EK-2 form per employee.

The form is completed by two parties. Workplace details and the employee's job description come from the employer, while the medical sections and the conclusion belong to the occupational physician. The sections are covered in order below.

1. Workplace details

Company name, social security registration number, address, hazard class and headcount are entered. The hazard class shapes the rest of the form, because the examination interval and the tests to be requested are planned by class. The class verified from the NACE code is used.

2. Employee details and job description

Identity details, date of birth, education, hire date and the job to be performed are entered. The most neglected field is the job description. Writing only worker is not enough. A term that shows the exposure is needed, such as welder, forklift operator or paint shop worker. The physician chooses the tests from this line.

3. Medical history

This section relies on the employee's statement. Past illnesses, operations, regular medication, allergies, smoking and alcohol habits, exposures in previous jobs and any history of occupational disease are recorded. The physician completes it in the presence of the employee.

4. Physical examination

It starts with height, weight, blood pressure and pulse. The respiratory, circulatory, digestive, nervous, musculoskeletal, eye, ear nose throat and skin systems are examined in turn. Systems without findings are marked normal, findings are written out clearly.

5. Laboratory and test findings

Biological analyses, radiological examinations and physiological tests are recorded here. Blood and urine tests, chest X-ray, audiometry, pulmonary function test and ECG results are entered with dates and values. Every test performed in the mobile screening supports this section.

6. Opinion and conclusion

The legal weight of the form sits in this section. The physician marks one of three conclusions. Fit, fit with conditions or unfit. For fit with conditions the conditions are written out, for example no night shifts, no work at height or mandatory use of personal protective equipment. Whether the examination is pre-employment or periodic is marked and the form is dated.

Retention and confidentiality

  • The EK-2 form is kept in the employee's personal health file. The file is retained for at least 15 years after the employee leaves.
  • For work with carcinogens and mutagens, records are kept for 40 years after the exposure ends.
  • The employer sees only the conclusion. Medical findings are the occupational physician's responsibility and count as special category personal data under the KVKK.
  • When the employee moves to another workplace, the new employer requests the file in writing and the previous employer sends a certified copy within one month.

Pre-inspection checklist

  • Does every employee have a current EK-2 form, and has any periodic interval lapsed.
  • Does the job description line reflect the real exposure.
  • Are test results entered on the form and report printouts added to the file.
  • For fit with conditions, are the conditions written and applied at the workplace.
  • Are the physician's signature, diploma number and stamp complete.
